*A SSL cert is required and a CSR will be provided when the FQDN is confirmed.

*It's optional  to request a name of your choice but it must follow our <customer>.<app>.rtvision.com format (Eg. the <customer> part). If not provided we will default to our regular <state>-<name>.<app>.rtvision.com format. Name availability is first come first serve.
